As a Social Worker with Evolent, you will serve as a critical member of the integrated care team, which is made up of registered nurses, pharmacists, registered dieticians, and program coordinators.

The Social Worker role at Evolent will provide both direct care management to a high-risk population, as well as provide clinical consultation to other members of the care team. Evolent partners with the nation’s leading health systems in their strategic evolution to positively impact and improve the healthcare delivery system. As a Social Work Care Advisor with Evolent, you will have the opportunity to make a profound impact on the lives of people living with multiple chronic illnesses and people who are uninsured with medical, behavioral and substance abuse issues.

What You’ll Be Doing:

  • Engaging patients and care givers in active care planning to focus on behavioral, clinical, social, and environmental concerns and needs of patients.

  • Create and maintain a comprehensive inventory of local community resources, improving accessibility for patients and providers, and linking patients with the appropriate support services.

  • Assess the patient’s knowledge of their clinical condition and provides education and self-management support based on the patient’s preferences.

  • Unite patients with primary care, behavioral health, respite, and other services.

  • Hybrid and flexible work schedule.

  • Conduct home visits with patients to assist with SDoH barriers or other needs identified by the CM team as needed

  • Help patient access community and governmental based service agencies including possibly filling out paperwork with/for the patient.

The Experience You’ll Need (Required):

  • Licensed Social Worker (LSW) master’s degree.

  • 2-3 years of Medical or Community Social Work experience providing patient-centered outreach, behavioral health services, needs assessment and support.

  • Demonstrate excellent interactive communication skills, the ability to engage with varying patient populations, and meet the patient ‘where they are’.

  • Be personally and professionally accountable for best-practice patient care.

  • Ability to travel to various locations.

Finishing Touches (Preferred):

  • Master of Social Workers (MSW) applicants are encouraged to apply.

  • Demonstrate the ability to work independently, as well as partner with multidisciplinary teams and health care providers. Advocate for patients within the medical and community settings.

  • Additional qualities that would be a good fit for our team include excitement and passion for helping patients, genuine spirit, kind, and thoughtful nature, and one who embraces a ‘go with the flow’ mentality.

  • Experience with Motivational Interviewing, Self-Management and Patient Engagement strategies are a plus.

  • Experience with Medicaid, insurance providers and telephonic patient engagement is a plus.

  • Bilingual English (Plus but not required)

  • Demonstrated interest in working with an underserved population.

Technical Requirements:

We require that all employees have the following technical capability at their home: High speed internet over 10 Mbps and, specifically for all call center employees, the ability to plug in directly to the home internet router. These at-home technical requirements are subject to change with any scheduled re-opening of our office locations.

The expected base salary/wage range for this position is $75,000. This position is also eligible for a bonus component that would be dependent on pre-defined performance factors. As part of our total compensation package, Evolent is proud to offer comprehensive benefits (including health insurance benefits) to qualifying employees. All compensation determinations are based on the skills and experience required for the position and commensurate with experience of selected individuals, which may vary above and below the stated amounts.
